We currently utilize Formstack to build and embed forms, collect data, register participants for events, and complete internal communications. While we originally started using Formstack specifically to collect data from our volunteers, we quickly saw the potential to implement it organizaton-wide. Before we elected to use Formstack we used some free tools such as Google forms. While that met our basic need we were not able to develop forms that met our branding guidelines. Formstack proved to be a great option for customization.
  • Formstack forms are very easy to build. The User Interface takes merely a few moments to figure out. Truly a drag and drop solution.
  • Advanced features such as logic questions and A/B testing are seamlessly integrated.
  • Their customer service is outstanding.
  • Great admin portal for those managing account.
  • The price point on the upper-tiers is a little high for what you get.
  • Include a live chat feature for customer support.
Formstack is functional, easy to use, and really helps organizations get organized. I find myself referring people to Formstack all of the time, which to me, is a great indicator of how well they address a customers need in their space. If you are looking for a tool that will help you build professional online forms, Formstack should be the first and last place you look.

Embedded forms for multiple use cases such as newsletter subscription, lead generation, scholarship applications, evaluations, etc. The ease of creating forms in a user-friendly interface that is also supported with a high level of security, user authentication, and data protection was important in the decision to go with Formstack. It was so easy to train new staff to use and even create options like custom dropdown lists, logic branching, checkboxes for GDPR compliance, etc. Reuse and duplication of forms was easy, as was customizing the look and feel for the embedded form depending on the host website branding. Being able to easily edit text in a form without complex tickets or lengthy wait times enabled the team to respond to user behavior such as low completion rates. We were also able to easily test the effect of A/B testing, changing up form field types, etc. by paying close attention to the analytics. While I personally did not use partial completes, that seemed like an attractive feature that stood out over the competition. We did encounter some issues with the forms not displaying correctly on mobile but that may have been an issue with the site it was embedded on. With consistent testing, we were able to bypass this. issue.

We contracted with Formstack to help us run a large annual survey project that required more ability to build in page and question logic and better reporting and analytics than the free online tool we had previously used. We have discussed rolling out Formstack to our staff generally, but have not done so to this point due to the relative ease of the free online tool we use and the simplicity of most of our surveys.

  • When discussing the scope of engagement, Formstack indicated a partnership with another company that would allow us to aggregate the survey data with our internal data. That partnership was oversold.
  • We were looking for a way to capture data in a table form. They could accommodate a small table, but not a large one, resulting in us having to modify the layout.
There is no question that we gained significant functionality in terms of analytics and page and question logic by choosing Formstack over the free survey tool we had used previously. The feedback from users who had taken the survey was that it was user-friendly and looked much more professional. There were some discussions in the sales phase that did not accurately represent the functionality, and while they were extremely accommodating to our changes based on user experience, and training was phenomenal, it wasn't conveyed at point of contract that the service we were provided as part of that contract was limited to an hour of edits, and that our additional time was at an additional cost. Finally, the fact that they connected us with another software that was supposed to make our data integration easier made it appear that they either didn't understand the scope of our project request or they didn't have a good handle on the other company's capabilities.

Cairn University uses Formstack for the majority of our online forms - from simple event registrations to our Application for Admission and everything in between. Formstack has been an integral part of our initiative to migrate many of our forms and applications to the web, which has - among many other things - greatly increased the speed at which we are able to move students through the enrollment process.
  • Very intuitive and simple use, while offering a lot of advanced features that offer a lot of control.
  • The conditional logic features allow us to build complex forms that walk users through a process, dynamically showing or hiding fields based on their previous selections.

If you have a need for advanced forms with conditional logic and payment integrations, Formstack is a great pick. Likewise, if you need a much more robust solution with front-end user accounts (to login and complete a form) or bulletproof integrations with an existing CMS, you might want to find a different dedicated enterprise solution. Formstack can fill in a pretty wide space between the two ends of the spectrum.

We use Formstack for a variety of needs where information is required to process requests such as purchasing, IT support and professional development requests. It is used mostly by operations but IT uses it as well. It allows us to keep track of all types of requests easily and provide feedback to users.

We used Formstack as part of our customer onboarding process, so primarily within a product and marketing context. We wanted to survey our new free trial users to segment them and deliver the most relevant training content possible. We embedded a multi-step survey after the free trial signup that branched according to certain high-level responses. This allowed us to better understand who was signing up for our product and how we could help learn our platform.
  • Formstack allowed us to customize the look and feel of our forms to match our branding. It was easy for our engineers to integrate the forms within our onboarding process to deliver a seamless experience.
  • Formstack integrates well with HubSpot, our marketing automation platform of choice.
  • Formstack offered us a lot of flexibility in the types/formats of questions we could deploy (open-ended text, multi-choice, single choice, etc.). This made it easy to get data the way we wanted and facilitated getting that data into HubSpot.
  • Setting up our multi-step form took some getting used to. Some of the branching logic configuration wasn't as intuitive as would be ideal.
  • Since we were pushing data directly into Hubspot, I found it cumbersome to deal with data storage limits within Formstack. I would get frequent notices that new responses were not being stored in Formstack and that I needed to upgrade or delete data. I got tired of that.
  • More/easier support for custom fonts would have been a huge plus. We were able to customize the form's appearance, but had to make some concessions in that regard.
Formstack is great for creating lead generation forms since it integrates so well with third party marketing automation platforms. You get more options for your forms than are standard with, say, HubSpot. One of the drawbacks that you need to be aware of is regarding design. If you are very concerned about the forms being able to match very specific branding and typography, you will not get what you need. Custom fonts are possible, but it takes more engineering work than is ideal. If you are a small team or don't have development resources to help you set it up, you're gonna have to compromise on the visuals.

Formstack is being used across our entire university. It is our enterprise form creation and workflow tool used by various departments across the campus to create forms and workflow.
  • Easy & intuitive form builder that allows users to create forms
  • Branding of forms with your company logo
  • Logic within forms to show and hide fields
  • Logic with email triggers to only send notifications when certain answers are filled out in the form
  • Their workflow feature is still half-baked. Lacks enhanced features like logic & ending the workflow mid-step
  • Some of their integrations are outdated and no longer supported so basically useless to our organization: ie.: Zendesk Integration
  • No integration with Active Directory so user management is a pain

We use Formstack in all 6 schools and at all levels of Administration. We use it for simple tasks like surveys and minor purchases, to complex forms like a 20-course, logic based summer program. We integrate with Google Drive, Authorize.net and a few other components. Its helped us move off of paper, reduce the need to collect cash, and automate many processes. We use it to build Help Desk tickets, and trigger workflows. We have nearly 300 forms, with approximately half of those active at any given time.
  • Conditional Logic: Formstack beats most form editors when it comes to "if, this, then let that happen"
  • Integrations: Formstack integrates with a ton of third party applications, and if Formstack doesn't directly, then its Zapier integration probably does
  • Embed: while its not the prettiest form output, it does embed nearly anywhere
  • Buggy Updates: Formstack tends to make on-the-fly updates that break features. We run into this about once a year
  • Appearance: the forms are starting to look dated compared to some of the competitors (looking at you, Typeform)
  • Dynamic Data: passing data from one field to another is not-exactly doable
  • Paid Add-Ons: the price for Formstack is already an arm and a leg; the fact that they have additional monthly paid add-ons is frustrating
  • User Limits: user accounts barely do anything outside of 'view/edit' permissions, thus the low volume allocated seems a bit greedy at $10/user/mo for more
Formstack is great at most things in bulk. It supports multiple users, lots of forms, lots of submissions, lots of addons, and lots of lots - you get the point. If you're looking to get a bunch of content off of dated systems, and into an online system/email workflow/crm, etc., Formstack will do just that.

What it won't do - it won't wow the end user with a beautiful form, though you can skin it to look native to your website.

Good for Midsize to Enterprise, doesn't need a programmer: Aside from the semi-complex Wordpress integration, Formstack is really easy to use. Secretary types without coding experience would have no problem building a form and sharing it. Technically minded folk will enjoy the complex conditional logic and the support it provides with third party addons and payment systems.

Bad for small businesses: Formstack's not economical, and with the budget-friendly competition, most small businesses are out. Formstack does a good job at complex integrations and spends a sizeable amount of effort on its paid addons rather than improving its core product, so unless you plan on spending a lot monthly and need a bunch of forms, you probably can look elsewhere (Gravity Forms, Ninja Forms, etc).

Our web production department utilizes FormStack for almost any case where we need an outside web form to be embedded on one of the websites we work with.

We support many different frameworks and CMS solutions. Some of them have nice form solutions built in, and others don't. None of them are very consistent with each other. FormStack solves that, and wherever we have a web platform that doesn't have a good form system, we build out that site's contact forms on FormStack and embed them.
  • Allows the visual building of generic forms that can easily fit into any website and that don't have to be created by a web developer.
  • Offers fairly advanced conditions and automation scenarios that can occur, allowing for some complex form logic.
  • Allows a choice of themes as well as the ability to override the form styling in your site's own CSS.
  • The themes that come with FormStack don't look particularly good to me. We usually end up going with the most plain-looking one so that it's easiest to override with CSS, but it would be nice if there were really generic themes that would better fit in with an existing site and look more modern without as much CSS overriding being required.
  • You can do a lot with the form logic in FormStack, but if you're used to having full control of the form output in a server language such as PHP, there it can be a big change having to think about things in terms of what you're able to accomplish through the FormStack UI. There are a lot of options, but it will probably never be as flexible as actually building a form yourself (but it sure is easier!).
  • Internally within a form, the layout options are pretty limited. We have a lot of cases where we have custom-designed forms that are supposed to have some fields next to each other, or multiple columns of fields, etc. In those cases, using custom CSS to try and make those changes is usually the best answer, but it would be great if a non-developer had more layout options for form fields.
If you need a generic, embedded solution for forms, where you can keep all of your forms and form responses in one place (Even for multiple clients), FormStack deserves a serious look. If you're comfortable using CSS to help style the forms how you want, then FormStack might just be the ideal solution for you.

Formstack is used by our whole organization. We use Formstack to handle all of our feedback forms, surveys, and inter-departmental requests. We've tried other survey and form services that could tackle part of our needs but not all of them. Formstack is ideal since it can easily be embedded into our website, integrates with other services we use and has robust email features.
  • Conditional Logic: we haven't found another service that does this quite as well as Formstack.
  • Simplicity/User-Friendly: Incredibly easy to build even the most robust of forms.
  • Embedding.
  • Integrations: Integrates with tons of other services/platforms that keeps our ecosystem running smoothly.
  • Sometimes the form builder is slightly clunky or buggy.
  • Complicated conditional-logic gets hard to follow/troubleshoot.
  • Seems like there is unnecessary feature or design switches that come out of nowhere.
Formstack is very well suited for surveys and feedback forms. We haven't really found a limitation or a place that it isn't appropriate.

It is being used on a limited basis to automate workflows for tasks like audit requests, system access removals, etc.

We currently use Formstack across the whole organization, as well as our congregation. Since we are a church, we can literally have thousands of users filling out forms. Formstack allows us to store information all in one place and allows for a notification process to happen internally for our staff. We create forms for human resource needs such as employment applications, and well as internal processes such as purchase and check requests.

It seems Formstack is more suited to internal processes. We do not use Portals, so that is perhaps best suited for external users.

We use Formstack for every area of ministry in our church, from event registration and payment to online donations. It may be a simple RSVP or signup form, or a complicated, logic-driven event registration. Formstack is easy to use and helps me to be able to craft sign-up forms for every application.
  • The conditional logic built into Formstack helps me craft forms and registrations with multiple options. It's pretty easy to pick up on how to use.
  • Mobile friendly forms that can be used directly in our app is a big plus.
  • The submission stats and charts have been invaluable to me in planning and keeping those in leadership informed.
  • I'd like to see Formstack be able to send each registrant a custom QR code for quick check in.
Formstack is well suited for times when options need to be available for people to choose. I can create a simple form with our online givine/CMS system, but there is no flexibility there in asking for options and especially with conditional logic. We do a music theatre camp each summer and offer optional extra childcare times before and after for a fee. Formstack makes it easy for parents to sign up and select all their options.

We use Formstack to gather data from our employees and customers. We use it across several departments, including Marketing, Sales, Finance, and HR. Formstack helps us collect important information and funnel it to the right people, organize it for detailed reporting purposes, and make good decisions based on the facts.
  • Formstack has a very robust form-builder that allows multiple field types, conditional logic, and calculations.
  • Formstack has a huge number of supported integrations to get your data where it needs to be.
  • Formstack's database of submissions allows for a lot of detailed reporting to be done on collected data.
  • It can be a little tricky to modify a form's appearance with Themes. This has gotten better with some recent updates, but there are still some issues on the appearance of forms that aren't perfect.
  • It's not always clear what users will see after they submit a form.
  • It would be great if Formstack supported an on page confirmation instead of redirecting to another page once forms are submitted.
We have used Formstack in a variety of ways, but it is extremely helpful for us in our Product Claims Management processes. We've built a very large multi-page form to collect data from our customers if they are having a problem with one of our products. We use a lot of conditional logic, field calculations, and alternate values to get detailed information. We then use Formstack's great integrations to create reports and summaries of the data on the fly that are sent to appropriate people based on the information collected in the form.

Formstack is used across the entire organization, i.e., anyone who needs a form can request and get one. We handle event signups, requests for college services, feedback, and general questionnaires. Our users utilize both email to receive submissions but also the database export to further utilize submission data. Others use 3rd party integrations like ZenDesk and Mailchimp.

Formstack essentially replaced our IT department's role of creating custom forms and database solutions, so they can focus on more appropriate and important tasks.
  • Ease of setup - we can create something almost immediately.
  • Conditional logic - enables the shortest form possible to be presented to the user, so forms can be comprehensive, but users only see the fields they need.
  • Themes - allow us to make a form look exactly how we want them to look.
  • Embedding - forms easily embed into our website, making the presentation seamless.
  • Integrations - allow our users to harness submission data in the ways that they need.
  • Sometimes how to integrate with 3rd parties isn't super obvious.
Our Registrar has a request form that has 26 separate scenarios, each with custom notification and confirmation emails. The notification emails go to at least 10 different employees in the Registrar. The ability to have a single form handle vastly different requests, provide both custom info inside of all emails, delivered to the appropriate employees is critical to their success as a department.

Our International Admission department has a pre-app form to capture interested international students as they begin our complex application process. The department is able to help guide them through that process by exporting their names from Formstack and comparing that list to an internal list of incomplete applications. They also integrate the form with Mailchimp to be able to communicate deadlines, important College information, etc.

We use Formstack on behalf of 16 associations that we manage. Uses range from collecting information for awards, volunteering, event RSVP, event registration with payments, small conferences and membership sign up and renewal. Formstack itself is mainly managed by the technology staff who provides other staff access to View/Edit and exporting.
  • We manage several smaller associations who have recurring events. Combining this with Stripe allows us to embed a professional looking form onto an existing website without having to pay for a third party event registration platform.
  • Reusable templates, ability to copy forms, fields and sections allow us to deploy recurring forms quickly and accurately.
  • 3rd party integrations solves many of our problems such as payment gateway (Stripe), coupons and reviews. We use reviews for membership applications which we really find very useful.
  • The logic feature is great but it would be an improvement to allow logic on a per item level with radio buttons, select boxes and check marks.
  • Working with themes can be cumbersome. I also don't think limiting themes is a good idea. It's an essential part of ones toolbox when designing a form. I get frustrated when I'm forced to delete one in order to create a new one.
Formstack is well suited for collecting information for events which require a more complex selection of fields and logic. You won't get this sort of flexibility with standard event registration systems. It's also very well suited for collection nomination information and biography profiles. It might not be well suited in situations where tier pricing and access levels are required.

We use Formstack across the entire institution however mainly we use it to automate and localize some important business processes localized Student Admission and Evaluation. It has been the best to automatize our Evaluation and Admissions processes replacing other high cost systems.

The approving and other workflow logic are useful in streamlining other business processes.

I have presented our model of the admission process to companies selling Admission software or plugins with the request to provide a simple and easier model they could now. Since our administration body is very small simplicity and ease of use for our students and administration is key.

We use Formstack to capture product registrations, support requests and repair authorizations. Formstack also has an API that allows us to easily send form submissions to our customer support portal, Zendesk. Formstack allows us to easily retrieve customer data and assist customers with possible concerns.

Formstack was used by the marketing department where I used to work. I use Formstack for all of our lead capture across our whole website, but mainly the contact us page. Since starting with Formstack over several years ago, I've seen an average lead conversion rate of around 20% -- most are marketing-qualified leads.
  • Formstack is [easy] to use. For example, building forms is a breeze.
  • I love that it integrates seamlessly with Unbounce and regular .PHP sites to give us quality, high converting lead pages.
  • Dashboard stats are a bit rudimentary but do a nice job giving a birds-eye view of lead capture progress.
  • Different types of form formats are available which is great.
  • Form stylizing on your website, without deep knowledge of CSS, can be a bit tricky.
  • Dashboard is a bit unimpressive; needs UX/UI improvements.
  • Integration APIs are a bit limited and its web hook function (if employed) can be a bit intimidating to set up the first time.
Depending on what your lead capture goals are, this a good solid, entry level option for building and maintaining lead capture forms. If you're considering Formstack, I would recommend signing on for a free trial first so you can play around in it and see how easy it is to create forms and how your team could use the product most effectively. Feel free to make your own form templates with your brand's colors to really visualize how the forms could look and function.

We currently use Formstack as a way to track and collect information for various internal business processes including industry relations, IT, and event planning. We also utilize the various integrations including Google Docs, Box and Salesforce.
  • Ease of use in building forms.

  • The Box integration is touchy.
  • Custom themes can get tricky.
  • Adding logic on email submissions would be nice.
Formstack has provided a user friendly solution for data collection and form building for our organization. The primary gain for us has been the ability to very quickly and easily build new forms and collect data to share with other members of our organization without investing a great deal of resources into it.

I have used Formstack with multiple organizations I have volunteered and consulted with over the past five years. Some of the uses include:

  • Event registration data collection and payments with PayPal for High Performance driving events
  • Event registration and payment collection for various charitable and non-profit events
  • Fannie Mae form 1003 data collection for the loan origination process
  • Small merchandise sales and order fulfillment
  • Salesforce and general 'form' data collection and request processes
In all but one case, each of my use cases and experience with Formstack has been connecting and integrating it with WordPress sites. My driving reason for using Formstack was the ability to easily connect to WordPress and begin the collecting and routing of end-user data.
  • Formstack integrates very well with WordPress. A simple plug-in install and any WordPress site is ready to begin collecting data.
  • Formstack has a plethora of options for the actual routing of data. Not only can you store data in the FormStack cloud for future retrieval, you have the ability to route the data to your own email, others email (for confirmation), or integrate with Google Docs
  • The management and flow process of forms is quite advanced. From calculations to conditional based logic, Formstack is capable of quite advanced, quasi-'systems' type data routing. This makes data collecting and the conditional collecting of data (i.e. if this, then collect that) very easy
  • Formstack is missing what I might consider some major integrations: it has Salesforce, but no NetSuite; it has Google Apps, but no Office 365
  • While the data routing options are impressive, one area that would be incredible would be the integration or data feeding of third party databases. For example, if data collected could be pushed to another cloud-based on on-premise PostgreSQL or Microsoft SQL Server instance, the ability to use the collected data with other integrated systems would be incredible.
  • In my opinion, Formstack can be a little pricey in the tiers. For not much money you can get into the service, but it's easy to quickly run out of space and need to jump to a more expensive tier
Formstack is best suited for (1) customer-facing, web-based form collection and (2) advanced form functionality. If your company is a subscriber to Google Apps or Office 365, basic form functionality is provided as part of the service. However, in both of the cases listed above, the functionality and options are nowhere near what is available through Formstack.

Said another way, if you need to quickly collect shirt sizes for the employees of your organization, there are plenty of easy to use web-based form mechanisms. But if you need conditional routing or are sensitive to the look-and-feel of your data collection, Formstack is the way to go. It is possible to seamlessly connect to your web site where customers will not even know you are using a third party data collector.

Several years ago, we grew tired of using development time to create what were, in most cases, simple forms. Although some divisions of the University would use Survey Monkey from time to time, we wanted to choose a product that was simple enough to use that would serve most of the usual needs around campus. We were impressed with the simplicity of Formstack's product for our end users, but also appreciated their integration modules, service and pricing. We currently have over 800 forms that have been created. Having a tool available for the entire Butler community has normalized our form-building processes and freed up technical resources for more challenging projects. The credit card integration was a huge win as well. It is no longer "rocket science" to get a department setup with credit card processing, even for smaller initiatives. The Authorize.net integration enabled us to take what used to be a major headache and turn it into a no-brainer. We've also appreciated how Formstack has involved us in their feedback loop and listened to our suggestions for product improvement. They are also a local company, which is an added bonus. We get to support a local vendor and can meet face-to-face if it is ever required.
  • Simple interface. They have created the tool to enable form building via several different methods, depending on how a person wishes to work. You can choose elements, then modify them to add them to a form. Or, you can simply drag and drop them where you want them. You can even choose from templates or a wizard to get you started. The tool couldn't be simpler to use.
  • Reliability. We can't think of a single time that Formstack has not been available. With extremely rare exception, any time we have a submission or process issue, it's usually due to something we've done incorrectly or because one of our systems dropped the ball.
  • The good people at Formstack seem to genuinely care about making their product better all the time. We are told about or shown new features, or proposed features, several times a year. In fact, they will often let us experiment with features before they unleash it system wide. They value our feedback and we feel that we've actually played a small part in shaping what Formstack has become.
  • Price. Compared to other form tools, Formstack is very reasonably priced. The analysis and reporting tools are not nearly as robust as some other large, extensible survey products. But, if you're just trying to get out of the form business and provide a solid platform to your users for building forms and surveys, this is a good option.
  • Online "stores." Formstack did away with their store module some time ago and replaced it with a form field type that is really better suited to events than products. If you're trying to build out an entire online catalog of products that may have various colors, sizes, styles, etc. Formstack is NOT the tool to use. However, if you just have a FEW items you wish to make available for sale, that can be done fairly well with a little effort and know-how.
  • Although Formstack is infinitely more customizable style-wise with it's graphic templates and CSS tools (much more so than, say, Google Forms), it is sometimes challenging to get granular control over certain form elements--even WITH a CSS guru on staff. As long as you're willing to say "close enough" and not get TOO picky, you can certainly create beautiful forms and surveys that complement your branding or graphic design elements.
  • Reporting and analytics. If you're looking for Qualtrics-like reporting, you won't find that here. You can certainly get base stats such as usage, geolocation, aggregate survey results, abandonment points and comparative graphs. But if you're a real data nerd who wants microscopic data analysis, you'll likely be disappointed.
As with anything, ask yourself: "What am I hoping to accomplish with this tool?" Be thorough. Be honest. If you're looking for Survey Monkey, get Survey Monkey. If you're looking for Qualtrics, get Qualtrics. If you're needing to empower your user base to create and manage their own forms (and even share that data with a few non-users) without all the complications that come from a more expensive tool, then give Formstack a try. With Formstack's ability to do conditional items and now even provide some simple workflow (approve/decline) processes, it is a powerful tool for many circumstances.

We used multiple instances of Formstack on the company website. I handled all our web design, so I was the primary user, but Formstack is simple enough anyone could use it. I created both simple comment forms and tech support requests, as well as more complicated financial applications.
  • Simple drag and drop creation makes it very easy to create and reorganize forms.
  • Frequently used fields are already available, but you can create any field you need in seconds.
  • You can have a form that combines single field questions, with multiple choice or even conditional logic.
  • You can create A and B versions of your form and analytics give you an easy way to assess which form is getting a better response.
  • The forms are very easy to brand.
  • I never ran into any problems using formstack. I only wish the rest of plugins were this user friendly.
Formstack is well-suited to a small business website. We were able to create forms for customer service requests, applications, general questions and for polls. We were able to get it to do all the things we wanted.

We utilized Formstack as a print submission tool. I suggested Formstack as more user friendly alternative to the Xerox XJT submission and management tools that we were initially utilizing when I came on board. The order form that I created was tailored to specific departmental requirements and used across the entire organization, including both our internal and external offices/personnel.
  • Formstack's conditional logic allows the form manager to create multiple click-paths for end-users to navigate per their specific order or inquiry. If set up properly, this allows for a less befuddling experience for the client/customer by displaying only pertinent options.
  • The interface at both ends is intuitive and requires no coding knowledge.
  • The API continues to evolve. Formstack is continually adding features that are quick-and-easy to integrate.
  • The customer support is far from stellar but not terrible. Also, Formstack should offer around-the-clock support.
  • The payment process is a bit messy.
  • The API is quite buggy in Internet Explorer.
Formstack is an excellent tool for creating quick and clean survey, inquiry, and order forms. The click-path is also easily customisable which is an asset if you have multiple end users utilizing the same form but with disparate requirements.
